3. From private limited → unlimited company.
4. From unlimited private → private limited company.
5. From public company → unlimited private company.
6. Expedited conversion of public company → private company.
The form records:
1. Current and proposed company status.
2. Company name before and after conversion.
3. Details of secretaries (where required).
4. Statement of guarantee (for conversions into companies limited by guarantee).
5. Member details and consents.
6. Lodging party information.
7. Attachments required (resolutions, amended articles, auditor’s statements, etc.).
